Add Active/Inactive option for Membership Add-ons

As membership programs are periodically revised, add-ons may be discontinued, and the option to inactivate retired add-ons would exclude that value from further transactions, yet retain its description text within the Membership Program Prices tab.  Currently the choices are to rename with "do not use" in the value name or to delete from the membership program.
