Add a Name on Card option to the Add a Membership drop down menu

In the Add a Membership window, we would like there to be a dropdown that captures all the "Name on card" options in a similar way to the drop down when editing the membership.

Currently there is a section titled "Which members should receive a card?" Options include "member," "Name on card," and "Expiration date." the Name on card section is an editable text box and not a drop down. We have to type out the names again (and possibly make input errors)
